Online version of XSV has experimental enforcement of 'all' groups

This has been an embarassing gap for some time -- I've used my new
counter stuff to implement proper enforcement of <xs:all>, which
heretofore was just implemented as a *ed choice.

Please report any unexpected behaviour to me ASAP.
